Donations to the Foundation through the Doctors Hospital “Tree of Life”
Located in the main lobby of Affinity Medical Center in Massillon, Ohio, is a replica of a tree consisting of gold leaves. Each leaf is engraved with the name or names of those who have made a $500 or more contribution to the Austin-Bailey Health and Wellness Foundation, which was formed when the not-for-profit hospital was sold to a for-profit organization in late 1996. Donations of all amounts are gladly accepted, but only contributions of $500 or more will be recognized with an engraved leaf on the Tree of Life. Those contributing $500 or more who want to add an engraved leaf should contact Don Sultzbach at 330-580-2380 or send an e-mail to firstname.lastname@example.org.
All donations are used by the foundation to promote the physical and mental well-being of the citizens of Holmes, Stark, Tuscarawas, and Wayne Counties, and to enhance the education of people in the healthcare field.
Since the Austin-Bailey Health and Wellness Foundation is a non-profit 501(c)(3) organization and is tax-exempt, your contribution will be tax-deductible.
