To support a part-time Registered Nurse for the Newark Community Integrated Services Project (“Newark CISP”) which was formed when the New Jersey Children’s Health Project was brought together with the Jordan and Harris Community Health Center to provide coordinated, case managed primary healthcare to underserved communities in Newark. Through a nurse managed community center at 3 public housing developments and a nurse-managed mobile healthcare initiative, the Newark CISP healthcare team conducts primary care, free general screenings and referrals, case management, vaccinations and other health promotions and education to approximately 1,600 patients annually. By bringing comprehensive primary care directly to the community, they aim to decrease Emergency Room visits and educate families about the value of having an ongoing relationship with a primary care provider. The primary role of the RN is to maintain an ongoing presence in the community where the population develops trust and freely engages with the RN and community healthcare workers.
